Smile & Oral Health Assessment

Terms & Conditions

1. Nature of the Service

1.1. The Smile & Oral Health Assessment (₱15,000) is a diagnostic and planning service.

1.2. It does not include actual clinical treatment (e.g., fillings, braces installation, PST surgery, crowns). These will be quoted separately after the Assessment.

1.3. Completing the Assessment does not guarantee that any particular treatment (e.g., braces, PST, implants) will be recommended or performed. The final plan depends on clinical findings and the dentist’s professional judgment.

2. Inclusions & Exclusions

2.1. The ₱15,000 fee includes:

  • Digital 3D intraoral scan

  • Clinical photographs

  • Comprehensive Teeth & Gum Examination

  • Oral Function Assessment

  • TMJ/bite/function analysis

  • Digital Bite Analysis (T-Scan – when clinically indicated)

  • Periapical X-Rays (when clinically indicated)

  • Personalized Treatment Planning & Consultation (with Written Plan)

The ₱15,000 fee is for the complete Assessment and expert planning. Not every case needs every tool (like T-Scan or multiple periapicals), but the process and professional analysis are the same.

2.2. The fee does not include:

Additional Tests like:

Panoramic and Cephalometric X-Rays
Ordered only when needed, coordinated with our partner diagnostic center and paid         separately at their rates.
If you already have recent, good-quality X-rays, you may bring them and we will use them where appropriate.

Blood tests, medical clearances, or referrals to other doctors

Any actual treatments (fillings, root canals, surgery, braces, FMR, PST, etc.)

3. Booking, Payment & Rescheduling

3.1. The Assessment fee is payable upon booking or on the day of the Assessment, as advised by the clinic.

3.2. Payment confirms the schedule.

3.3. Rescheduling is allowed with at least [24 HOURS] notice, subject to availability.

3.4. Late cancellations or no-shows may result in partial or full forfeiture of the fee, at the clinic’s discretion.

3.5. If the clinic cancels and no alternative schedule is acceptable, the Assessment fee will be refunded.

4. Existing X-Rays / Diagnostics

4.1. Patients are encouraged to bring any recent X-rays or diagnostic records they already have.

4.2. If the quality and recency are acceptable, the clinic may use these existing records as part of the Assessment to avoid unnecessary repetition.

4.3. The Assessment fee remains ₱15,000, as the main value is in the comprehensive evaluation, analysis, and planning.

4.4. If existing X-rays are unusable or incomplete, the dentist may recommend new X-rays, which may incur additional cost if outside the included set.

5. 2026 Smile Priority Plan (Rate-Lock Conditions)

To qualify for the 2026 Smile Priority Plan (rate-lock at 2025 rates), ALL of the following conditions must be met:

5.1. The Smile & Oral Health Assessment is completed on or before January 31, 2026.

5.2. The Reservation Fee of ₱[AMOUNT] (or as low as [_5_]% of the total treatment fee) for the agreed Treatment Plan is paid on or before March 31, 2026.

5.3. The planned treatment is started on or before June 30, 2026, defined as the first major clinical appointment related to the plan (e.g., braces/aligner installation, TMJ appliance delivery, first FMR tooth preparation, or PST procedure).

5.4. The rate-lock applies to the professional fees of the procedures specifically listed in the written Treatment Plan presented after the Assessment (e.g., orthodontic, TMJ therapy, FMR restorations, PST procedures).

5.5. The rate-lock does not apply to:

Future new problems (new decay, trauma, infections, new tooth loss)

Routine maintenance (e.g., standard cleanings, perio maintenance) unless explicitly included

Elective cosmetic add-ons (e.g., extra veneers, whitening, derma)

Replacement of lost or damaged appliances beyond the original plan

Increases in external costs (labs, implants, special materials), where price changes are driven by third-party suppliers

5.6. If there is a significant change in the patient’s clinical condition after the Assessment (e.g., new tooth fractures, progression of gum disease), the Treatment Plan and fees may need to be revised in line with the current condition and clinic rates.

6. Reservation Fee

6.1. The Reservation Fee is deductible from the total treatment cost once the planned treatment begins.

6.2. The Reservation Fee is non-transferable to another person.

6.3. It is generally non-refundable, except if:

The clinic determines that treatment should not proceed for clinical/medical reasons, or

The clinic cancels the planned treatment and the patient chooses not to reschedule.

7. Program Changes

7.1. Oraderm may modify, extend, or discontinue the Smile & Oral Health Assessment and the 2026 Smile Priority Plan in the future.

7.2. Any changes will not affect patients who have already completed their Assessment, signed their Treatment Plan, and paid their Reservation Fee under previously agreed terms.
